Prostate cancer ablation with a 4D robotic system using thermal ultrasonic waves under MRI guidance

SUMMARY
PROSTASONIC addresses the major social challenge of treatment of prostate cancer using magnetic resonance guided focused ultrasound (MRgFUS). The goal is to produce an advanced prototype robotic system with 4 degrees of freedom (DOF) that can ablate prostate tissue using MRgFUS. The intended application is to use this system for focal ablation of prostate cancer. The tissue heating can be accurately monitored using MR thermometry. The proposed single element transducer will be compact (around 20 mm in diameter) and will operate with a frequency close to 3 MHz. A software will be developed that will control this medical device. The system will be evaluated in phantoms, excised tissue and animals. With the proposed disrupting technology, we hope to change the culture of prostate cancer therapies. This system can be applied in the future for other prostate applications (for example Benign prostate hyperplasia).

SUMMARY
FUSROBOT’s main goal is to produce an MRI-guided focused ultrasound (MRgFUS) robotic system for preclinical use of small and large animals. MRgFUS is becoming a very hot and attractive non-invasive modality for oncology (cancer), and neurology (Parkinson). Therefore, the number of research institutions involved in this area is growing rapidly. There is urgent need for these research institutions for affordable and functional preclinical MRgFUS robotic systems in order to explore new applications in MRgFUS. The goal is to produce a preclinical robotic system (final product) with 4 degrees of freedom (DOF) that can sonicate phantoms, excised tissue or animals using MRgFUS. The tissue heating can be accurately monitored using MR thermometry. The ultrasonic system will include a single element transducer (ranging from 20-60 mm in diameter) and will operate with a frequency ranging from 0.5 to 4 MHz. A software will be developed that will control this medical device. The system will be evaluated in phantoms, excised tissue and animals. By the end of the program all the necessary documents for patent application will be ready.

BRAINSONIC

Treatment of brain tumors using therapeutic ultrasound

SUMMARY
BRAINSONIC is a program for optimizing MRI signal and image for thermal ultrasonic surgery of brain tumors. Basically the contrast to noise ratio using Fast spin echo (FSE) MRI sequences was evaluated. This system was tested initially in freshly excised lamb brain. A spherically focused transducer operating at 1 MHz was developed. The transducer was navigated using an MRI compatible robot. Two important issues were tested: a) Transmission of ultrasound through the intact skull and b) Opening of the Blood brain barrier using low intensity ultrasound. The experimental verification was performed in rabbit brain. Throughout the project, the effects of ultrasound were evaluated using histology.

ULTRASTROKE

SUMMARY
ULTRASTROKE is a program for optimizing MRI signal and image ultrasonic treatment of Stroke. This program emphasized in upgrading equipment infrastructure. One major equipment that was purchased is a digital manufacturing system (FDM 400 from STRATASYS) which produces plastic structures made out of ABS.

PROFUS

Development of an MR-guided Focused Ultrasound device for the treatment of Prostate cancer

SUMMARY
The objective of PROFUS was to develop and evaluate clinically an innovative medical device, using focused ultrasound under MR imaging control, to provide an alternative, non invasive method to treat prostate cancer effectively and reliably without the complications of surgery.

SONOPLAQUE

Destruction of atherosclerotic plaque using intense ultrasound

SUMMARY
In SONOPLAQUE thermal and mechanical therapeutic ultrasound was evaluated for removing atherosclerotic plaque in vitro and in vivo(rabbit) model. A parametric analysis was performed to obtain the optimum frequency of the transducer. The transducer was evaluated in MRI for its functionality. Plaque was produced in rabbit using a high cholesterol diet for 3 months. The combination of intense ultrasound and microbubbles efficiently removed plaque from the aorta of rabbits.

SONOMRI

High intensity focused ultrasound surgery of brain tumors


This program was funded by the Ministry of Trade, Commerce and Tourism of the Cyprus between 2005 and 2008 (170,000 Euros). The goal of this program was to develop an MRI compatible positioning device for treating brain cancer using high intensity focused ultrasound. A very important aspect of this project was the optimization of contrast between brain and necrotic tissue.
